CK Community Health Centres Launch New Strategic Plan

Chatham-Kent’s multi-service health centres have launched a new five-year strategic plan with the goal of providing the best possible health and well-being for those in the community.

Chatham-Kent Community Health Centres has implemented its 2026-2031 strategic plan effective April 1, following an extensive community engagement process.

The plan’s four strategic priorities are connections, organizational vitality, equity and inclusivity and service excellence, which also align with the system priorities of the Chatham-Kent Ontario Health Team.

More than 100 respondents engaged in focus groups, surveys, interviews, planning meetings, and discussions as part of the community engagement process to inform the strategic plan.

CKCHC also re-aligned its organizational values of compassion, accountability, respect, equity and safety to better reflect its commitment to person-centred care, community wellbeing, and a culture of trust and inclusion.
