Family Health Teams Recognized

Family Health Teams across Chatham-Kent are coming together to share data and determine how best to help patients.

The CK, Thamesview, and Tilbury District Family Health Teams have been working to improve the lives of patents that have been heavy users of hospitals and emergency rooms, to the benefit of all.

Denise Waddick, Executive Director for the Thamesview FHT, says by following high needs patients through the system, they’re able to track what sorts of treatments work.

“So not only are we saving dollars for the government in terms of acute care usage and hospital costs,” Waddick explains, “we’re also providing much better care and keeping our patients healthier and at home longer.”

For their work, the health teams are receiving provincial recognition in the form of a “Bright Lights” award from the Association of Family Health Teams of Ontario.
