The two organizations have been working to launch a community paramedicine program that helps with the transition between hospitals and home care providers.
The program is being recognized for making that transition as easy as possible for patients and families.
That frees up emergency care services for other patients.
The CKOHT is a group of 15 health care providers working together and was founded just three months ago.
