CKHA Makes It Easier To Go Home

Let’s be honest, the best part of any hospital stay is when you are released. But that can be a confusing and involved process. Chatham-Kent health alliance is working to make it easier.

CKHA is taking part in a new program called PODS. Patient Oriented Discharge Summaries. It’s a new tool designed to help patients better manage their own care when they are released.

Discharge documents are often long, and full of medical jargon most people don’t understand anyway. If you can’t understand the instructions that won’t help the recovery process.

In the case of CKHA they deal with nearly 9,000 patient discharges yearly so they are trying the program co-developed by University health Networks Open Lab.
