The Chatham-Kent Ontario Health Team is looking to increase its list of partners.

The organization was formed in December 2019 with over a dozen member agencies, including hospital, home, and community care organizations.

Three years later, the group is hoping to add more health care partners to its roster with a goal of providing seamless, fully coordinated care for patients, while creating a more efficient health care system.

The OHT’s original priority population included those aged 55 and older with chronic conditions such as COPD, heart failure, dementia, diabetes, or others requiring complex or palliative care. Now, the organization is expanding its priorities to include those living with mental illness and/or addictions.

An information seminar will be held on Friday, January 20 from 12-1pm on Zoom. Video from a previous webinar held December 8 can be found on the CK-OHT website at www.ckoht.ca
