The Chatham-Kent Community Health Centre is looking to upgrade its van to an accessible vehicle, and recently received a big boost from Farm Credit Canada’s Agri Spirit Fund.

The CK-CHC has received a check for $15,000 to go towards the new vehicle.

It allows the Age Well Program to help residents across the municipality with home safety assessments, develop individual exercise plans and provide health education for chronic disease management.

“With limited funding for infrastructure renewal, we rely on funding from sources like the FCC AgriSpirit Fund to deliver care to those we serve throughout Chatham-Kent,” said Sherri Saunders, Executive Director. “We are excited that we were a successful recipient of this funding that will allow us to replace our aging van to be able to continue supporting clients in the community.”

The Age Well Program staff includes a registered nurse, and a physio and occupational therapist.
