As a local health collective prepares for the next round of applications to create an Ontario Health Team, officials are narrowing their focus of care.
Health Alliance CEO says the Chatham-Kent Ontario Health Team’s application passed the self-assessment evaluation, and is one of 31 groups across the province to be invited to submit a full application.
“All of our partners are thrilled to be among the first group of applicants who will proceed to the next phase in forming an Ontario Health Team,” Marshall says. “In Chatham-Kent, we have an already established track record of collaboration, innovation and integration between local providers that’s focused on delivering patient-centred care and seamless transitions between providers.”
The organizations, which include the Health Alliance, local Family Health Teams, Hospice, and Community Health Centres, among others, will be focusing on care for older adults with chronic diseases and complex needs.
Future plans will be expanded to include all patient populations.
The deadline for submissions to the province is early October.
